Wisconsin’s medical board governs the physician’s side of a delegation.
Whoever performs a delegated medical act in Wisconsin, the supervising physician remains answerable for it under the adequate-supervision test — which reaches licensed and unlicensed personnel alike. A nurse carrying out a delegated act is therefore working inside the physician’s duty, not outside it.

Wisconsin grants the physician assistant the act itself, in words that reach an injection directly. Subject to the limitations in subsection (2), and to the physician assistant’s own experience, education and training, a physician assistant may “Examine into the fact, condition, or cause of human health or disease, or treat, operate, prescribe, or advise for the same, by any means or instrumentality” and may “Penetrate, pierce, or sever the tissues of a human being.” On drugs the grant is equally direct: “consistent with his or her experience, education, and training”, “a physician assistant may order, prescribe, procure, dispense, and administer prescription drugs, medical devices, services, and supplies.”

The limit is a named physician, not a supervision ratio. a physician assistant who provides care to patients must maintain, and provide to the board on request, evidence that pursuant to the physician assistant’s employment “there is a physician who is primarily responsible for the overall direction and management of the physician assistant’s professional activities and for assuring that the services provided by the physician assistant are medically appropriate” — or, in the alternative the statute offers, a written agreement. “Employment” here expressly includes an arrangement in which a third party receives payment for the physician assistant’s services.
